Complete our Patient Intake Packet before you come into the office by printing and completing the following forms: Patient Information, No Show Policy, and HIPAA Privacy Rights forms before your appointment and bring it with you to the office. If you would like to have your child's records released to you, please complete this Medical Records Release form, and return to our office.

Please note, all insured and non-insured patients will be charged a $50.00 fee for any missed appointments, or cancellations and rescheduling with less than 24 hour notice.

We accept most insurance and Medicaid. Please contact your insurance provider to check network status with Bentonville Pediatrics, PA.
